PERSONAL INFORMATION
Visitors (“Visitors”) may visit the Website without providing personal data to us. SECURE PATIENT PORTAL
As a convenience to its patients, Jenkins Clinic offers access to the co-branded Secure Patient Portal, operated by Intuit, Inc.  Please note that the Secure Patient Portal is provided to you by Intuit, Inc., not by Jenkins Clinic.  Intuit, Inc. provides the Secure Patient Portal to you as a Business Associate to the Jenkins Clinic under its Intuit Health Subscription Agreement.  As a Business Associate, Intuit, Inc. is subject to the rules that apply to Business Associates under HIPAA and the Health Information Technology Act of 2009 (“HITECH Act”).  In order to utilize the Secure Patient Portal, you must agree to the Intuit Health Terms of Service.  The Secure Patient Portal is not under the control of Jenkins Clinic and Jenkins Clinic does not assume any responsibility for the operation, content, privacy practices of the Secure Patient Portal or the technology it implements. You agree that you use the Secure Patient Portal at your sole risk. Jenkins Clinic in its sole discretion may remove the link to the Secure Patient Portal at any time and without notice.

USE OF HEALTH INFORMATION
In addition, you should be aware that Jenkins Clinic may use and disclose PHI as permitted by applicable law, including the Privacy Rules under the Health Insurance Portability and Accountability Act of 1996. See the Notice of Privacy Practices for Patient Information for more detail.

NON-PERSONAL INFORMATION
Jenkins Clinic gathers information about how Visitors use our Website. We may track visits to our Website for trends and statistics, but Visitors will remain anonymous unless they specifically identify themselves to us.
