Please note that the Notice of Privacy Practices for Patient Information is a separate document that governs how Jenkins Clinic treats its patients’ Protected Health Information (“PHI”), as that term is defined in the Health Insurance Portability & Accountability Act (“HIPAA”).
Visitors (“Visitors”) may visit the Website without providing personal data to us. However, certain sections of the Website may include contact information that Visitors may use to direct inquiries or feedback to us, such as by email. In such cases, we may use the email address you provide to respond to your inquiry or otherwise fulfill your request.
Jenkins Clinic may disclose specific personal information upon governmental request, in response to a court order, when required by law, to enforce our Website policies, or to protect Jenkins Clinic’s or others’ rights, property, or safety. Jenkins Clinic does not provide personal information to these entities for marketing or commercial purposes.
SECURE PATIENT PORTAL
As a convenience to its patients, Jenkins Clinic offers access to the co-branded Secure Patient Portal, operated by Intuit, Inc. Please note that the Secure Patient Portal is provided to you by Intuit, Inc., not by Jenkins Clinic. Intuit, Inc. provides the Secure Patient Portal to you as a Business Associate to the Jenkins Clinic under its Intuit Health Subscription Agreement. As a Business Associate, Intuit, Inc. is subject to the rules that apply to Business Associates under HIPAA and the Health Information Technology Act of 2009 (“HITECH Act”). In order to utilize the Secure Patient Portal, you must agree to the Intuit Health Terms of Service. The Secure Patient Portal is not under the control of Jenkins Clinic and Jenkins Clinic does not assume any responsibility for the operation, content, privacy practices of the Secure Patient Portal or the technology it implements. You agree that you use the Secure Patient Portal at your sole risk. Jenkins Clinic in its sole discretion may remove the link to the Secure Patient Portal at any time and without notice.
USE OF HEALTH INFORMATION
In addition, you should be aware that Jenkins Clinic may use and disclose PHI as permitted by applicable law, including the Privacy Rules under the Health Insurance Portability and Accountability Act of 1996. See the Notice of Privacy Practices for Patient Information for more detail.
Jenkins Clinic gathers information about how Visitors use our Website. We may track visits to our Website for trends and statistics, but Visitors will remain anonymous unless they specifically identify themselves to us. Of course, a Visitor who chooses to give us personal information is no longer anonymous to Jenkins Clinic.
